Home-Based Family Support Program

The Home-Based Case Manager meets the “total” needs of the child through guiding and assisting parents or guardians in gaining access to necessary medical, educational, rehabilitative and social services. Parents are informed regarding what to expect in child development along with teaching the family the benefits of a healthy lifestyle with accident and disease prevention.
